Up-to-date information about travel health and immunisation recommendations and requirements for travel can be found on the Department of Health Travel Advice website.
Travel immunisations can be arranged with the Practice Nurses. We keep a stock of most vaccines at the surgery.

You will need to complete a travel form prior to your appointment, which can be obtained from reception, or you can print this out (please click on the Travel Vaccination Form below). This will enable the nurse to review your records and make an assessment before your appointment (advice for some areas changes regularly). Please complete ONE form for EACH person travelling and either bring in person or fax to the practice prior to your appointment.
We currently do not provide the following vaccinations and you will need to go to an alternative clinic – charges may vary between providers:
- Yellow Fever vaccination (you will need to go to an accredited yellow fever centre)
- Rabies vaccination
- Japanese Encephalitis vaccination
